We make the referral process simple.

Refer a Patient

Healthcare providers and referring offices can contact our team to coordinate Hyperbaric Oxygen Therapy for their patients. We assist with clinical documentation requirements, insurance verification, scheduling, and coordination of care.

Ready to refer a patient? Call our office at 480-590-5277 or fax one of the provided referral forms, along with supporting medical records and clinical notes, to 480-590-6145. Once received, our team will contact the patient directly to assist with scheduling and next steps.

Insurance & Self-Pay

We accept Medicare and select commercial insurance plans, including Cigna and UnitedHealthcare, for qualifying medical conditions. Our team can assist with insurance verification, eligibility, coverage requirements, and available self-pay options.

Same-Day Appointments

Same-day appointments may be available. If we're unable to accommodate you the same day, we'll work with you to find the earliest available appointment that fits your needs.
